M2 Discuss how the practitioners and agencies involved in the care pathways work together to provide the care needed for both physiological disorders Introduction In this assignment I will discuss the effectiveness of health and social care practitioners and agencies working together to deliver the care pathway for one of the chosen physiological disorders. The disorder I have chosen to evaluate is diabetes type 2 diabetes. Strengths Working in a multidisciplinary team means the patient with type 2 diabetes can receive a better all-round level of care. All ideas and treatment plans can be discussed weighing up the advantages and disadvantages of each situation. For example, a nurse might suggest something that would be disadvantageous to a person’s occupational needs. It is also less likely that things are missed, as more than one person is involved. From an operational point of view, all team members are aware of progress and major decision meaning if sickness or annual leave happens the patients care does not stop. Individuals that have been diagnosed with type 2 diabetes are cared for by a variety of people like podiatrist, district nurses, DSNs, GPs, and practice nurses. Good communication between these professionals and the person with type 2 diabetes can lead to better insight into the individual’s life, therefore helping to plan and provide the best care for that person.
